Do you know about “Multilingual Medical Questionnaire”？
This is a questionnaire translated into multiple languages.
You can use it for explaining your symptoms of illness or injury to your doctor
when visiting a Japanese hospital.
It can be downloaded from our website free of charge.
It covers 11 medical departments, including internal medicine and surgery,
and each department has its own questionnaire.
If you are not sure which department to visit, you can also search by symptoms.
