- [IKE18-39] "Moving-out Form (Tenshutsu Todoke)" and "Moving-in Form (Tennyu Todoke)"

When you are moving, you need to submit a "Moving-out Form" to the municipal office where you live.
Submission is required when you are moving out of Japan as well.
A "Moving-in Form" should be submitted within 14 days to the municipal office where you will be residing.
Procedures to change the address on your "Individual Number (MY NUMBER) Card" or
"Notification Card" is also necessary.
■What to bring:
・Identification documents of the submitting person
・Original "Moving-out Certificate" issued at the municipal of your previous residence
・"Notification Card" or " Individual Number (MY NUMBER) Card" of the “MY NUMBER” Social Security and Tax Number System, or "Basic Resident Register Card"
・Residence Card or Special Permanent Resident Certificate
・Power of Attorney, if someone will submit on your behalf
・National Health Insurance Card and such
