- [IKE21-28]【COVID-19 Vaccination Certificate (Vaccination Passport)】

There are countries where quarantine measures are relaxed when entering, if you present proof that you have been vaccinated against COVID-19 abroad.
In Japan, the Vaccination Certificate (Vaccination Passport) is issued by municipalities as official proof of vaccination against COVID-19.
Eligibility:
The Vaccination Certificate will be issued to persons who fulfill both of the following conditions.
（１）Those who have been vaccinated using the vaccination vouchers or coupons issued by the municipality they live in.
（２）Those who are scheduled to travel overseas.
Where to apply:
The municipality which sent you the vaccination voucher
Necessary documents for application:
（１）Application form (available at your municipality).
（２）Passport (Passports issued by foreign governments can be used for the application.
If your passport number is changed after receiving your vaccination certificate, you need to apply for a new certificate with a new passport number).
（３）Individually numbered “Vaccination Voucher (接種券)” with two vaccination stickers removed. (If you have lost it, you need to present a document that shows your “Individual Number,” such as your “My Number Card” or certificate of residence with your individual number on it).
（４） “(Temporary) Certificate of Vaccination for COVID-19 (接種済証)” or “Record of Vaccination for COVID-19 (接種記録書)” which you received after vaccination.
（If you have lost it, you may use a copy of your “Pre-Vaccination Screening Questionnaire.”

Information included in the Vaccination Certificate (Vaccination Passport):
・The name, date of birth, etc. of the vaccinated person
・Record of vaccination, such as the type of vaccine and the date of vaccination
・Passport number
